New Patient Information
What can I expect when I arrive?
As you enter, you will be greeted and welcomed as a member of our family. We will ask you to fill out a New Patient Information Form. Click here to preview or print: http://preview-4.baystonemedia.com/userfiles/11798/pdf/manion_new_patient.pdf.
This paperwork provides us with information about you and your condition. Next, one of our Chiropractic Assistants will discuss your health related problems and concerns. Your first visit is designed for us to learn more about you, your condition and your expectations to determine whether chiropractic care can meet your goals. We will also perform a complete chiropractic examination to test your posture, balance and range of motion. Your specific condition may require us to take x-rays or order diagnostic studies to help us rule out a more serious condition or assist us in developing the most effective treatment plan for you. Most people are amazed once they see their x-rays and can often identify their misalignments and degeneration themselves.
What's Next? Report of Findings
Dr. Manion will review your history and your goals with you.. He will then explain what your x-rays reveal about the condition of your spine.
Once all the information and examinations have been performed, Dr. Manion will give you a report of findings to answer the three most popular questions.
1.Can you help me?
- 2. What do you recommend that I do to get better?
- 3. How much is it going to cost?
- Finally, Dr. Manion will discuss recommendations with you. If Dr. Manion believes chiropractic care can help you, he will recommend treatment and a wellness program.
What information should I bring with me to my appointment?
